Standard Visits for Immunizations

2 Month - Pediarix (DTAP, HEP B, IPV), Prevnar, HIB, Rotavirus

4 Month - Pediarix (DTAP, HEP B, IPV), Prevnar, HIB, Rotavirus

6 Month - Pediarix (DTAP, HEP B, IPV), Prevnar, Rotavirus, *Flu

12 Month - HEP A, MMR, Varivax, Prevnar

15 Month - Pentacel (DTAP, HIB, IPV)

18 Month - HEP A

4 Year - Kinrix (DTAP,IPV), Proquad (MMR, Varivax)

11 Year - TDAP, Menveo, *HPV

16 Year - Menveo, *MEN B

​* HPV is a 2 dose series vaccine if administered before the age of 15. If administered after the age of 15, it becomes a 3 dose series. 

​* MEN B is a 2 dose series vaccine given 1 month apart. 

​​* Flu vaccine is offered to all patients at the beginning of each flu season. It may be a 1 dose vaccine or a 2 dose (given 1 month apart) based on age.
