Advanced Care Team

​Our Advanced Care Team is a team of specialist that coordinate and work together to ensure the highest quality of care is provided throughout our practice. ​​

RN Triage -provides triage services by phone and in our office when needed.​

​

Telehealth/RN Coordinator -acts as liaison between provider and patients to schedule appointments for telehealth services and follow up on previous telehealth appointments.

LPN Care Coordinator - provides case management services when needed and assists with coordination of care and follow up.  

Health Coach - Medical assistant and staff members to review immunization rates and provide patient appointments to keep patients current.
